West Lakes Family Dentistry is proud to offer our uninsured patients an in office discount program. The goal of the program is to assist more patients/families in achieving optimal dental health by providing alternative means to cover preventive care.

The Care Club at West Lakes is an annual reduced fee dental plan. The benefits/savings associated with the discount plan are non-transferable to another patient, dental office or specialty practice.

Covered services of membership:

• 2 professional cleanings
• 2 clinical exams
• 1 set of digital x-rays
• 2 professional fluoride treatments
• Plus 1 emergency/limited oral evaluation with x-rays

The Care Club plan is not a dental insurance plan and cannot but used to pay for/in conjunction with the following:

• Another dental plan or dental insurance
• Treatments deemed outside the realm of dentistry
• Dental care covered under automobile or medical insurance
• Dental care associated with a worker’s compensation claim
• Dental care completed at a specialty practice
• Costs of goods sold within the dental practice (Electric toothbrushes, whitening materials, etc.)

Membership details:

• Membership is good for 1 year from date of enrollment
• Membership dues are payable in full at date of enrollment
• Membership dues are non-refundable
• Payments for additional services beyond preventive care covered with membership are due at date of service in full
• Acceptable payment methods are cash, check and credit card
• Memberships must be current to receive discount
• Fee associated with dental services may change at any time
• Membership can be cancelled at any time with written notice, membership dues are non-refundable
• Please notify office at least 48 hours in advance of any changes required to scheduled appointment as not to incur fee for cancellation/failed appointment
• No deductibles, pre-authorizations, yearly maximums or waiting periods are associated with membership
• No exclusions for elective/cosmetic treatment/care
